Admissions Information

We provide various services to adults with intellectual and developmental disabilities providing that the potential participant meets all of the following criteria:

• The consumer is at least 18 years of age
• The consumer’s Individual Program Plan (IPP) includes an Objective(s) that can be met by an adult
• The consumer's individual program plan (IPP) includes objectives that can be met through our services

If your are interested in placement for your consumer, please review the procedures below and contact us to begin the process.

Admissions Procedure

• Case Manager , Care Provider, or Family inquires with Program Director
• Case Manager submits Information packet (See Complete Packet instruction below)
• Program Director invites Consumer, Family, Care Provider, Case Manager to tour facility.
• Program, ID Team, and Consultants make decision if consumer is appropriate for services offered by Program

A Complete Information Packet includes

• Current IPP
• Current Medical Info (i.e., diabetic, seizure disorder, heart problems, etc)
• Current CDER
• Current or previous day program’s ISP,IEP, Behavior Plan
• Current Annual Report and Behavior Plan if residing Residential Facility
• Current Nursing Report if residing in ICF
